CHA2DS2-VASc Score: How to Calculate It and Apply It

Calculate the CHA2DS2-VASc score line by line, compare 2023 ACC/AHA and 2024 ESC anticoagulation thresholds, and avoid the common scoring mistakes.

Do this

  1. Score age 65 to 74 as 1 and 75 or older as 2. The bands exclude each other.
  2. Anticoagulate at 2% or more annual risk: score 2 or higher in men, 3 or higher in women (ACC/AHA 2023).
  3. Choose a DOAC over warfarin unless moderate to severe rheumatic mitral stenosis or a mechanical valve is present.
  4. Under ESC 2024 CHA2DS2-VA, anticoagulate at 2 or more and consider it at 1.
  5. Count treated hypertension and heart failure with preserved ejection fraction when you score under ESC definitions.

Calculate the CHA2DS2-VASc score line by line in patients with atrial fibrillation (AF), then compare the total with the 2023 ACC/AHA/ACCP/HRS thresholds (sex-specific) or the 2024 ESC thresholds (sex-neutral). The two guidelines disagree on the female sex point.

How do you calculate the CHA2DS2-VASc score?

Give 2 points each for age 75 or older and for prior stroke, TIA or thromboembolism. Give 1 point each for heart failure, hypertension, diabetes, vascular disease, age 65 to 74, and female sex.

The 2023 ACC/AHA/ACCP/HRS guideline lists these components. The 2024 ESC guideline defines the same items for CHA2DS2-VA without the sex line.

Run the arithmetic with the CHA2DS2-VASc calculator. The older CHADS2 score omits vascular disease, age 65 to 74 and sex. In the 2010 derivation paper, CHA2DS2-VASc placed fewer patients in the intermediate group (15.1%) than CHADS2 (61.9%).

What score warrants anticoagulation under current guidelines?

Anticoagulate at an estimated annual thromboembolic risk of 2% or more under the 2023 US guideline, or at a CHA2DS2-VA of 2 or more under the 2024 ESC guideline. Both guidelines treat the next band down as a shared decision.

  • US 2023: Class 1, Level A at 2% or more per year (for example, 2 or higher in men, 3 or higher in women). Class 2a, Level A ("reasonable") at 1% to under 2%, equivalent to 1 in men and 2 in women.
  • DOAC over warfarin in eligible patients, meaning those without moderate to severe rheumatic mitral stenosis or a mechanical heart valve.
  • ESC 2024: recommended at 2 or more and considered at 1, using a patient-centred, shared approach. An ACC summary lists the same thresholds.

Why did the ESC drop the sex category?

The ESC task force calls female sex an age-dependent stroke risk modifier and proposes CHA2DS2-VA in the absence of locally validated alternatives. The US guideline keeps CHA2DS2-VASc, shifts women's thresholds up one point, and names female sex among the risk modifiers that future scores may incorporate better.

Most patients get the same answer from either guideline. A woman with a CHA2DS2-VASc of 3 has a CHA2DS2-VA of 2, and a man scoring 2 gets the same result under both. The frameworks diverge at the borderline: a woman with a CHA2DS2-VASc of 2 gets Class 2a "reasonable" under the US guideline, while her CHA2DS2-VA of 1 prompts "should be considered" under the ESC.

A retrospective cohort of 3,370 adults aged 65 to 90 with new-onset AF found that CHA2DS2-VA removed the sex gap in risk strata. Its authors state this does not show better prediction.

Which scoring mistakes come up most often?

Check age, sex and the three loosely defined conditions before you total the score. Each error can shift a patient across a threshold.

  • Age bands. Age 65 to 74 earns 1 point and 75 or older earns 2. Age 78 scores 2.
  • Female sex alone. A woman whose only point is sex scores 1, which sits below the US intermediate band (2 in women).
  • Hypertension. The ESC definition includes current treatment, so a well-controlled patient on antihypertensives still scores.
  • Heart failure. The ESC definition covers symptoms regardless of LVEF, so preserved ejection fraction counts.
  • Vascular disease. ESC lists coronary disease including prior MI, and peripheral vascular disease. The 2023 US guideline names the category only, so check your calculator's definition.

Where does this fall short?

The Lip 2010 abstract reports a C-statistic of 0.606, only marginally better than CHADS2. The 2023 US guideline notes that the score performs suboptimally in selected populations such as renal disease, omits AF characteristics like burden, and says bleeding risk scores should not be used in isolation in patients at high stroke risk. The ESC guideline says to consider other modifiers such as cancer, chronic kidney disease, ethnicity, troponin and BNP. As of 2026-10-10, a September 2026 guideline listing shows no AF stroke-prevention guideline newer than the 2024 ESC document, and the 2026 ACC/AHA AF update covers performance and quality measures without changing risk thresholds, so confirm against your society's site before you update protocols.
